Problem Based Learning via web-based simulations of clinical cases. Example for myocardial infarction.

In unison of the directives of the European Union and the recommendations of the World Health Organisation and the World Federation for Medical Education significant changes are taking place in medical education, namely introducing of problem based learning, integration of theory with practice, increasing the role of distance- and continuing-all-life learning.
The purpose of the present work is to prepare a Web-based simulation on a clinical case of myocardial infarction to be solved by pre-clinical medical students in Biochemistry (regular and distance learning). The problem is dedicated to the role of enzymes for diagnostics and treatment of myocardial infarction. First the problem is presented from a diagnostically point of view. Characteristic intracellular enzymes as creatinephosphokinase, transaminases, lactate dehydrogenase and some of their isozymes are accentuated, as well as their time appearance and compartmentalisation. Several items are also dedicated on enzymes, used for treatment such as streptokinase and tissue-plasminogen activator (alteplase).
The students acquire experience to proceed along the diagnostical process, to differentiate among important and misleading information, given by the computer. The.JavaScript programme allows following of several possible lines. On the basis of previous knowledge and acquired new one, the students make their own decisions -correct or incorrect. Even making mistakes, the students reach the correct diagnosis and solve the case. The students understand the role of the biochemical theory for the clinical practice and are seriously motivated to study biochemistry. At the end, the programme gives additional information, stimulating further knowledge and opening new horizons. This kind of learning is active and professionally oriented.
